The lens glass (lens model), i.e. "J16ax8B4" or "A15x8"

What is the intended sensor size for this glass?
This should be the only connector on the lens itself. Usually Hirose 12-pin for most B4 lenses.
The camera manufacturer and model that you successfully tested this lens on.

This is calculated automatically based on minimum focal length and zoom multiplier.
2x extenders essentially zooms the image in by 2x, which will double your zoom distance, but also help on cameras that experience some vignetting when using the lens.

If the final lens+camera assembly is somewhat interesting, you are free to provide an image of that.
